THE LAPAROSCOPICAL
GASTRIC SLEEVE

Gastric sleeve surgery is a restrictive procedure. The surgeon removes a large part of the stomach, which creates a smaller, tubular stomach. The capacity of the new stomach is limited to around 100-150 ml, while a normal stomach has a volume of around two to three liters.

Due to the small capacity of the new stomach, the patient can eat less and consequently reduces weight.

Sleeve gastrectomy has become one of the standard operations in bariatric surgery in recent years. The sleeve weight-loss surgery has several advantages over other bariatric procedures. On the one hand, no foreign materials are implanted, like gastric banding, and on the other hand, the anatomy of the gastrointestinal tract is preserved, which is why nutrient deficiencies generally do not pose a long-term problem with sleeve weight-loss surgery.

Current research also shows that gastric sleeve surgery also has other positive hormonal effects on the body. It has been proven that type II diabetes mellitus improves significantly after sleeve weight-loss surgery.

In addition, in contrast to gastric bypass, it is a less complex and accordingly lower-risk procedure. Our partner surgeons in bariatric surgery recommend gastric sleeve surgery in affected patients as an initial procedure with a body mass index of over 35 kg / m².

Operational procedure

The sleeve weight-loss surgery is a mainly restrictive procedure, but it also has a hormonal effect.

The gastric sleeve operation is performed laparoscopically, using the so-called keyhole technique, and is consequently minimally invasive. During the approximately 45 to 90-minute operation, trocars are passed over the abdominal wall into the abdomen. The stomach is detached from the environment and cut lengthways with the help of a stapler, and welded shut at the same time. In addition to the already secure staple seam, staple reinforcements are used, which ensure additional firm anchoring.

The separated area of the stomach includes not only the corpus (the largest part of the stomach) but also the gastric fundus, in which the appetite-stimulating hormone ghrelin is formed. With its removal, the feeling of hunger is significantly reduced.

The Stapler
The most important instrument in laparoscopic gastric surgery is the stapler, which is used as a suturing device to separate the stomach.

In addition to the tightness of the seams at the staple line, bleeding is the most common complication in bariatric surgery. Our partner surgeon uses the stapler only once per patient to minimize this risk

The Suture Enhancer
Our partner surgeon Dr. Mustafa Erol is on the use of the suture enhancement. These consist of a biological material and serve as an additional layer of tissue to ensure that the brackets are better anchored, thereby reducing postoperative complications.

There is 100% consensus between specialists in the bariatric field about that by using the suture enhancer there is a significantly minimized risk of bleeding.

Many surgeons need 4 to 5 trocars for this operation – our partner surgeon Dr. Mustafa Erol typically uses 3-4 trocars, which makes the procedure less invasive to the body.

Every surgical procedure is associated with risks. In addition to the general operational risks such as intolerance to anesthesia, there are special operational risks.

Leakage: All seams on the stomach or intestine have the risk of impaired healing with the development of a leak in this area and usually require a follow-up operation to eliminate this leakage, otherwise peritonitis can occur.
